Patient Disclosure & Usage Agreement
Version 1.2 · Effective September 29, 2026
AffordableHealth.ca is a directory and matching service, not a health-care provider. It is not for emergencies. You pay professionals directly. Read this before you register.
1. What AffordableHealth.ca is — and is not
AffordableHealth.ca ("the Platform") is an online directory and matching service operated by Homula Inc. ("we", "us"). We help connect people across Canada with independent, licensed health professionals ("Professionals") who have chosen to offer reduced or sliding-scale fees.
- We are not a health-care provider, clinic, or professional corporation.
- We do not provide medical advice, assessment, diagnosis, treatment, triage, or a second opinion.
- We do not employ the Professionals listed. They are independent practitioners responsible for their own services, records, insurance, licensing, and conduct.
- We do not guarantee that any Professional will accept your request, that an appointment will occur, or any particular outcome or price.
2. This is not for emergencies
Do not use AffordableHealth.ca in an emergency or crisis. If you or someone else is in danger, or you have a medical emergency, call 911 or go to your nearest emergency department. If you are in a mental-health crisis or having thoughts of suicide, call or text 988 (Suicide Crisis Helpline), available 24/7 across Canada. For non-urgent health advice in Ontario, call 811.
3. Eligibility
You must be at least 16 years old and located in Ontario to register. If you are registering on behalf of someone else (for example, a family member), you confirm you are authorized to do so and to share their information for the purpose of finding care.
4. The information you provide
When you make a request you tell us: a type of care, a general location, your preferred language, your budget range, an issue category, and a short free-text description of what you are looking for.
- Keep it general. Do not include your full medical history, diagnoses, medication lists, or sensitive clinical detail. A sentence or two about what you need is enough.
- The short description and your budget are shared only with Professionals you choose to contact, or — if you turn on "Available now" — with online Professionals in the relevant sector so they can offer to help.
- You are responsible for the accuracy of what you submit.
5. Budget, fees, and payment
- You set the minimum and maximum you can pay per visit. Professionals will not be shown to you if their stated floor is above your maximum, and they agree not to charge you more than the fee you both confirm in the Platform.
- You pay the Professional directly for your care, using whatever method they accept. AffordableHealth.ca does not process, hold, or handle payment for health services, and takes no commission on your care.
- Confirm the fee, what it covers, and the cancellation policy with the Professional before your appointment.
6. "Available now"
If you turn on "Available now", your request (sector, budget, and one-line description) is sent to Professionals who are online in that sector. This is a convenience feature for non-urgent situations only. It is limited to a set number of uses per day and each request expires automatically. A response is not guaranteed. Before every "Available now" request you must confirm you understand this is not an emergency service.
7. Verification of Professionals
We check each Professional’s registration against their regulatory College’s public register before their profile appears, and we re-check periodically. Verification confirms registration status only. It is not an endorsement, and it does not guarantee the quality, suitability, or safety of any service. You are responsible for deciding whether a Professional is right for you, and you can independently check any Professional on their College’s public register.
8. Reviews
You may leave a review only after a booking. Reviews must be honest, based on your own experience, and must not contain clinical details, personal attacks, or anything defamatory or unlawful. We may remove reviews that breach these rules.
9. Your responsibilities
- Provide accurate information and treat Professionals and our team respectfully.
- Do not use the Platform to harass, defraud, or seek prescriptions or controlled substances improperly.
- Keep contact and communication inside the Platform until an appointment is confirmed.
- Attend or cancel appointments in good time.
10. Limitation of liability
To the fullest extent permitted by law, AffordableHealth.ca is not liable for the acts, omissions, advice, treatment, fees, or conduct of any Professional, or for any loss arising from your use of the Platform or your dealings with a Professional. The Platform is provided "as is". Nothing in this agreement limits liability that cannot be limited by law.
11. Privacy
Our handling of your information is described in the Privacy Policy, which forms part of this agreement.
12. Changes
We may update this agreement. If we make a material change, we will ask you to review and accept the new version before you continue to use key features. The current version is always available at /legal/patient_agreement.
13. Contact
Questions about this agreement: use the contact form on the Platform.
By registering, you confirm that you have read, understood, and agree to this Patient Disclosure & Usage Agreement.
